Diagnosed with GIST cancer in Sept 2020. Recommended to take oral chemo and shrink the cancer down as much as possible, then remove it.

My case was not normal, as the cancer was so close to the rectum, the only operation would be a permanent colostomy.  Not ideal outcome for a Mid 30s male who was just about to be a father.  

Option 1 - keep the cancer, stay on the drugs without knowing an endpoint.  If the cancer spreads, it could spread fast.

Option 2 - remove the cancer, live with a colostomy for the rest of my life, and stay on the medication to prevent it from coming back with ~70% success rate at keeping it away 15+ years.

Not an easy choice, and the Doctors in Canada were salivating at doing the operation as this type of surgery is rare and all surgeons want that notch on their belt.

An old gambling buddy of mine suggested I look outside of the Canadian medical system to get at least a second opinion.  With a little help from someone who spoke portugese, I was able to get a video call with Dr Pestana in just under a month.  Once i got him my scans, the call was set up.  He spoke english, and gave me an honest opinion on the recommendations of my Canadian team.  I had some of the best oncologists across Canada working on my case, but a close person friend who's also a medical MD said sometimes methodologies within systems can be echo chamblers, so its good to hear how other places would approach this complicated situations.  While the recommendations were the same, the second opinion was the last check i needed to feel confident in my choice to pick surgery.

Dr Pestana even wanted me to follow up with him after my surgery and make sure there were no other questions i may have had.

I have been having pain in my shoulder since March 2024 (I am writing this post in January 2025). I got a cortisone shot in June and the symptoms came back in September. It got aggravated to severe pain by mid October, when movement got completely restricted. I got an MRI and it turns out I have a labrum/SLAP tear. I was in excruciating pain and the sports medicine doctor I was seeing gave me a second cortisone shot, which reduced the pain from a 9/10 to a 4/10, but the pain never went away completely. I have been doing a conservative treatment with a physiotherapist and a chiropractor, who are awesome. While these treatments have helped me manage the pain and improve the join stability by working the surrounding muscles, I have had three subluxations in a 40-day period. I was on vacation in Brazil in November when I had the first subluxation. I went to see Dr. Gobo, who did a maneuver to place my shoulder back in place and did a thorough physical evaluation. He suggested I get a second MRI because he said I presented rotator cuff-like issues in addition to the SLAP tear symptoms. I got an MRI the next day for 200 CAD (I paid 950 CAD in Calgary). The following day, the results were out and I saw him again. This guy not only worked a miracle to get the results in 24h, he also fit me into his busy schedule. It turns out my SLAP tear was complicated because there is a flap of cartilage flipped into the joint, and there is also a partial bicep tendon tear. Dr. Gobo called the imaging technician and discussed my case with him in front of me during the appointment. This doctor can legit interpret MRI images by himself, which none of my Canadian doctors did. He recommended surgery, which I am still deciding on. The appointment cost me 105 CAD. I will update this post as the case unfolds.

I struggled with unspecific symptoms for 4 years, such as tiredness, mild hair loss, and acne. My family doctor in Canada would ask for bits and pieces of blood work, and there was never an issue in my results of the tests he requested. I eventually stopped getting my menstrual periods. While on a trip to Brazil, I saw Dr. Cecilio in person, and she requested a huge list of blood tests to investigate what was going on. It cost about 500 CAD for 20+ very specific tests. It turns out I was in perimenopause. Dr. Cecilio did a hormone replacement therapy with intradermal implants, and less than 6 months later I was back to having normal levels of hormones and having menses again. A 1.5-hour appointment with her cost about 85 CAD. I am not sure she speaks fluent English, but she is highly trained in the field.

After tearing my achilles, I scheduled a Zoom meeting with Dr. Fonseca in under a week to get advice on the best recovery options for me (surgical vs. non-surgical). 
The meeting was about 30-45 minutes and cost about $100. He provided me with a PDF of a week-by-week rehab protocol that was very helpful.
It was incredible to get the advice of someone so highly accredited (https://drlucasfonseca.med.br/sobre-o-medico/) for such a low price.
I highly recommend him.
